We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: Pleurisy & heart Disease and that he receives a pension of (left blank) dollars per month. He makes the following statement upon which he bases his claim for (original, increase, restoration, etc.) Original.
Gets stitch in left side when stoops or lifts. Lasts few minutes only has it when lifts hard or quick jerk. breathing at times short especially on exertion. Heartbeats very fast; has (illegible)when sits down. Has pain about heart at (illegible). Feet swell at times. Spits blood at times. Has a bad stomach. Has trouble with gas; food lies heavy on stomach after eating. Is very nervous.
Upon examination we find the following objective conditions: Pulse rate 100; respiration, 21; temperature 98 1/2; height, 5feet 5inches; weight135 pounds; age 56years. "As to Pleurisy" Examination of lungs shows lower 2 inches of left lung is hepatized, dullness on percussive and entire absence of lung sounds. As to Disease of Heart. Examination of heart shows apex bent 1 in. below and 1 inch to right of nipple; hear tis enlarged. Hearts action rapid beats 120 standing and has mitral segargitation. Lips cyanotic. No a......on any part of body. Stomach lymparetic. Walls of stomach thickened and tender on pressure. Liver enlarged, spleen normal. Conjunctiva tinged. Tongue deep red and fissured. Body poorly nourished, general appearance anaemic. No other disabilities found.
He is in our opinion, entitled to a 6/18 rating for the disability caused by Disease of Left lung, 10/18 for that caused by Heart Disease, and 4/18 for that caused by Disease of Liver & Stomach.
