CLAIMANT AFFIDAVIT
State of Pennsylvania
County of Clarion
In the matter of claim of Joseph Mitchell, B-169-Pa Vols
Personally came before me, a Justice of the Peace in and for aforesaid County and State, Joseph Mitchell age 56 years, Kossuth P.O. residence, Ashland, who being duly sworn and is a person of lawful age, who being duly sworn, declare in relation to the aforesaid case as follows:
That to the best of my knowledge I contracted the diseases of left lung and liver and also disease of Stomache about the year 1889. It is impossible for me to state exact time or if these different troubles came on me at the same time or not and I further state that the above described disabilities are not due to my own vicious habits and to the best of my knowledge they are permanent.
Witnesses further declare that they have no interest in said case, and are not concerned in its prosecution.

Sworn to and subscribed before me this second day of April 1892; and I hereby certify that the contents of the foregoing affidavit were fully made known and explained to the affiant before swearing thereto, including the words "ninety one", erased, and the words "1889" added; that the affiant is to me well known and is respectable and worthy of full credit.; and I further certify that I have no interest, direct or indirect, in the prosecution of this claim. "A" Declaration for Original Invalid Pension
State of Pennsylvania
County of Clarion
On this 20 day of October A.D. one thousand eight hundred and eighty one personally appeared before me O. E. Nail, Orphan's Court Clerk, the same being a Court of Record of the County and State aforesaid, Joseph Mitchell, a resident of Ashland Twp. County of Clarion State of Pa. who being by me duly sworn according to law, on his solemn oath, deposes as follows, to wit:
"I am the identical Joseph Mitchell who was enrolled on the 16 day of Oct 1862 in Company B of the 169 Reg't of Pa Vol's, commanded by Captain Henry J. Shotts and I was honorably discharged at Harrisburg Pa on the 26 day of July 1863 and my age is now 46 years. While in the service aforesaid and in the line of my duty I received the following disability, to wit:
About the middle of May 1863 while at Gloucester Pt. Va. I contracted pleurisy of left side & heart disease caused by exposure. It still affects me & for the above I claim a pension.
Treated by regt. surg. in camp Gloucester Pt. Va.

I have never been employed in the Military or Naval Service of the United States otherwise than set forth above. Since leaving the Service, I have resided at Clarion Co. Pa and my occupation has been farmer; before my entry into the Service aforesaid I was of good sound physical health, being at enrollment a farmer and I am now partially disabled from obtaining my subsistence by manual labor by reason of my disabilities above stated, received in the service of the United States, and I make this Delcaration for the purpose of being placed on the Invalid Pension Roll of the United States. I hereby appoint and empower, with full power of substitution, N.W. Fitzgerald & Co, of Washington, D.C. my true and lawful Attorney to prosecute my claim. My Post Office address is Kossuth County of Clarion State of Pa".

Also personally appeared Moses Wise residing at Ashland Township Clarion Co. Pa and Daniel Starr residing at Ashland Township, Clarion Co. Pa., persons whom I certify to be respectable and entitled to credit, and who being by me first duly sworn according to law, say ther were present and saw Joseph Mitchell the claimant make his mark to the foregoing declaration; that they have every reason to believe from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be; and that they have no interest in this claim for Pension.
