DECLARATION FOR ORIGINAL INVALID PENSION. Under an act granting pensions to soldiers and sailors who are incapacitated for the performance of manual labor and providing for pensions to widows, minor children, and dependent parents.
State of Pennsylvania, County of Clarion.
On this 18 day of July A.D. one thousand eight hundred and ninety (left blank), personally appeared before me, a notary public, in and of the County and State aforesaid, Joseph Mitchell, aged 55 years, a resident of Kossuth, County of Clarion, State of (left blank), who being duly sworn according to law, declares that he is the identical Joseph Mitchell who entered service during the War of the Rebellion under the name of Joseph Mitchell on or about the 16 day of Oct, 1862, as Private in company B of the 169 regiment of Pa Vols commanded by (Henry?) Shotts Capt, and was HONORABLY DISCHARGED at Harrisburg on or about the 20 day of July, 1863, by reason of Expiration of Service; that his personal description is as follows: Age 29 years; height 5 feet 5 inches; complexion Sandy; hair, Sandy; eyes Bluer. That he is now suffering from Plueracy & Heart Disease. and that the said disability is of a permanent character, and is not the result of vicious habits, and that it incapacitates him from the performance of manual labor in such a degree as to render him unable to earn a support, and that this declaration is made for the purpose of being placed upon the pension roll, under the provisions of the Act of June 27, 1890. That he has not been employed in the military or naval service otherwise than as stated above. That since the 20 day of July A. D. 1863, he has not been employed in the military or naval service of the United States.
He hereby appoints, with full power of substitution and revocation, George E. Lemon of Washington, D. C., his true and lawful Attorney, to prosecute his claim, That he has never received but has applied for a pension. See No. 432246. That his Postoffice address is Kossuth, County of Clarion, State of Pennsylvania.

Also personally appeared Reuben Kline, residing at Kossuth Pa and Neri Boyer, residing at Mong, persons whom I certify to be respectable and entitled to credit and who, being by me duly sworn, say they were present and saw Joseph Mitchell the claimant sign his name to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be; and that they hav no interest in the prosecution of this claim.
