GENERAL AFFIDAVIT - Execute this before a Notary Public or some officer who uses a seal.

In the Matter of the Claim for Widdows Pension of Pheba A. Church widdow of Uzziel B. Church D 11 and J 16 ind Inf
Personally came before me, a county clerk in and for the County and State aforesaid Pheba A. Church aged 72, of No. [blank] St. Martinsville, Illinois, R. R. #7 and [blank] aged [blank] , of No. [blank] St. [blank] person of lawful age, who being duly sworn, declare in relation to the aforesaid claim, as follows:
That said claimant has no record showing the name and date of birth of all her father's family and that she does not know what became of her father's Bible which contained said record, and that to her knowledge there is no Jublie[?] or Church record of the date of her birth. That said claimant has always counter her birth date as November 14, 1845, and the place of her birth near Knightstown, Henry County, Indiana.
That she lived with her parents at the place of her birth in the summers of 1850 and 1860.
That William M. Overman, father, Jane E. Overman, mother, Nancy J., Mary A., Sarah and Margaret Overman, sisters; and Charles W., Robert T., Richard B., Samuel M., and James Overman, brothers, together with said claimant were all living in June 1850 and June 1860.
That Charles W. Overman, brother, served in the U. S. Army in the Civil War.
