		+	In the Matter of the Claim for Widdows Pension of Pheba A. Church
		+	widdow of Uzziel B. Church D 11 and J 16 ind Inf

		+	Personally came before me, a county clerk in and for the County and State aforesaid Pheba A. Church aged 72, of No. [blank] St. Martinsville, Illinois, R. R. #7
		+	and [blank] aged [blank] , of No. [blank] St. [blank]
		+	person of lawful age, who being duly sworn, declare in relation to the aforesaid claim, as follows:

		+	That said claimant has no record showing the name and date of birth of all her father's family and that she does not know what became of her father's Bible which contained
		+	said record, and that to her knowledge there is no Jublie[?] or Church record of the date of her birth. That said claimant has always counter her birth date as November
		+	14, 1845, and the place of her birth near Knightstown, Henry County, Indiana.
