		+	I, Fred Albert Dawson being of the age of 51 years hereby apply for membership in this Society by right of lineal descent in the following line from Timothy Church who was born in South Hadley, Mass. on the 12th day of May 1736 and died in Brattleboro, Vt. on the 13th day of Nov. 1823 and who assisted in establishing American Independence.

		+	(1) I am the son of Henry Lee Dawson born 2-28/1841, died 1/28/1923, and his whife Rhoda McFadden Dawson born 11/26/1862, died 9/27/1933, married 1883 or 1885.

		+	(2) grandson of George Dawson born about 1790, died 7/7/1845, and his wife Rebecca Church Dawson born about 1801, died about 1866, married 1831.

		+	(3) great-grandson of Samuel Church born about 1765, died after 1845, and his wife Elizabeth ? Church born , died , married prior to 1821.

		+	(4) great<sup>2</sup>-grandson of Timothy Church born 5/12/1736, died 11/13/1823, and his wife Abigail Church (Cousin) born 1738, died , married 6/9/1757.

		+	(5) great<sup>3</sup>-grandson of Nathaniel Church born 1704 died 1780, and his wife Rachel McCranny born , died 1788, married .

		+	(6) great<sup>4</sup>-grandson of Samuel Church born 1667, died 1737, and his wife born , died , married .

		+	(7) great<sup>5</sup>-grandson of Richard Church born 1649, died 1684, and his wife Mary Churchill born , died , married , and he, the said Timothy Church (No. 4) is the ancestor who assisted in establishing American Independence, while acting in the capacity of Lieut. Col. in the Revolution.

		+	NOTE: By Constitutional Amendment adopted May 17, 1927, Article III, Section 1, quoted below, was changed so that any man of 18 years of age is eligible to membership. Junior memberships have been abolished entirely.

		+	Applicant affirms his adherence to the principles set forth in the Declaration of Independence and his unqualified support of the Constitution of the United States of America, to the end that the inalienable rights of the citizen may be preserved and our ideal representative government perpetuated.

		+	"Section 1. Any man shall be eligible to membership in the Society who, being of the age of eighteen years or over, and a citizen of good repute in the community, is the lineal descendant of an ancestor who was at all times unfailing in his loyalty to, and rendered active service in, the cause of American Independence, either as an officer, soldier, seaman, marine, militiaman, or minute man, in the armed forces of the Continental Congress, or of any one of the several Colonies or States, or as a signer of the Declaration of Independence; or as a member of a Committee of Safety or Correspondence; or as a member of any Continental, Provincial, or Colonial Congress or Legislature; or as a recognized patriot who performed actual service by overt acts of resistance to the authority of Great Britain." -- Constitution of National Society S. A. R., Article III, Section 1, adopted June 5, 1907, and as amended.

		+	Membership is based upon one original claim; when the applicant derives eligibility by descent from more than one ancestor, and it is desired to take advantage thereof, separate applications, to be marked "Supplemental Application," should be made in each case and filed with the original.

		+	State fully such documentary or traditional authority as you found the following record upon, and also the residence of ancestors with dates of birth and death, if known.

		+	By order of the Board of Trustees, May 3, 1911, the recipient of a certificate granted on this application agrees to surrender his certificate upon failure to pay his membership dues, unless his membership shall cease by reason of death or actual disability, to be determined by the Society of which he is a member.

		+	My ancestor's services in assisting in the establishment of American Independence during the War of the Revolution were as follows:
		+
		+	Served as Second Lieut., Capt., and Lieut. Col. from Brattleboro, Vt., He was appointed Second Lieut. on January 4, 1776, by the Committee of Safety. He was than[sic] serving in the Brattleborough Co. Vt. On August 18, 1778[9?] he was made a Captain and in July 1782 he was appointed Lieut. Col. Of the Southern Regiment of Cumberland Co. Vt. by the Council of Appointment of N.Y. At that time there was a territorial dispute between New York and Vermont regarding Cumberland Co., each Colony claiming it as part of its territory.

		+	He is buried in Brattleboro, Bt. and his tombstone designates him as a Col. and says that he died November 13, 1833, at age 86. This appears to be a discrepency[sic] in other records which show his death as having been in 1823.
